Preplacement Appraisal Information

While the 602 form is completed by your physician, the 603 form — officially the Preplacement Appraisal — is yours to complete. Think of it as the family and individual’s counterpart to the physician’s report: where the 602 addresses your medical picture, the 603 captures the fuller, more personal one.

This form collects information about your health, physical and mental condition, functional capabilities, social factors, and specific care needs — ensuring that a community can develop a care plan tailored to you.  It covers everything from daily living needs and personal preferences to what kind of assistance, if any, you may require.

But the 603 is more than just a required document — it’s also a useful tool for families and individuals who are just beginning to think about senior living. The questions it asks are provocative in the best way: they nudge you to reflect on what matters to you socially, what your daily life looks like, and what kind of environment will truly feel like home. Many families find that working through the form together brings clarity and opens important conversations.

Like the 602, this form works in conjunction with the physician’s report rather than substituting for it  — both are required as part of the admission process for independent, assisted living and memory care communities.
